Government Authority located in Waterford, Michigan; USA based organization looking for expert vendor for substance abuse counseling services.
[A] Budget: Looking for proposal
[B] Scope of Service:
(1) Vendor needs to provide substance abuse counseling services to the government authority located in Waterford, MI.
- Provide case management services on each client until end of grant year.
- Weekly group counseling, individual counseling and relapse prevention services for 8 clients men and women until end of grant year.
- Provide updates, amendments or changes to a treatment plan and be willing to share with the team.
- Provide bi-weekly progress reports to the probation officer and shall discuss report contents with the team during review hearings.
- Attend bi-weekly or monthly review hearings, typically held from 8:00- 10:30am and 12:30pm to 4:30pm on Wednesdays.
- Be available by telephone or email to both the team and clients.
(2) A contract term will be for one year.
[C] Eligibility:
Onshore (USA Only)
[D] Work Performance:
Performance of the work will be Offsite. Vendor needs to carry work in their office premises.
