[A] Budget: Looking for Proposals
[B] Scope of Service:
(1) Vendor needs to provide occupational health services,
- Provider employs a board-certified physician or physician’s assistant to conduct the physical exam, including reviewing the job requirements.
- Provide can supply a comprehensive and complete history report for each preemployment physical.
- Provider can provide the following medical tests: audiogram, lift test, dot physical preplacement, and standard preplacement physical.
- Provider can offer the following drug screen options: regulated, non- regulated, rapid eCup and extended steroid panel.
- Provider can provide MRO services (in-house or third party).
- Provider must comply with the department of transportation regulations covering drug and alcohol testing, medical examination and report writing protocols and procedures to the government authority located in Ann Arbor, MI.
(2) All written questions must be submitted no later than August 28, 2018.
[C] Eligibility:
- Onshore (USA Only);
[D] Work Performance:
Performance of the work will be Offsite. Vendor needs to carry work in their office location.
