[A] Budget: Looking for Proposals
[B] Scope of Service:
(1) Vendor needs to provide relapse prevention support services includes sign in sheets and obtain participant signatures for all individual and group services rendered, meet individually with each potential participant to conduct a biopsychosocial assessment to determine program eligibility and needs, develop a clinical treatment plan for all accepted participants based on assessments, the approved treatment plan, gender-specific group counselling sessions focused on relapse prevention strategies and assist participants in successfully completing 8 group sessions to be released from the Program to the government authority located in Allegan, MI.
(2) All question must be submitted in written no later than August 16, 2018.
(3) A contract term will be one year.
