[A] Budget: Looking for proposal
[B] Scope of Service:
(1) Vendor needs to provide re-entry case management services to sign in sheets and obtain participant signatures for all individual and group services rendered, conduct a needs assessment on each referred potential participant as they are identified and submitted to assess their individual reentry needs, including potential substance abuse and mental health issues, assist participants in developing a written re-entry plan based on their short and long-term re-entry needs, assist the participant with the transition back into the community and provide transportation to the participant if needed and client advocate among the above support groups and provide guidance on how they can best support the participant to the government authority located in Allegan, MI.
(2) All question must be submitted no later than August 16, 2018.

Onshore (USA Only)
[D] Work Performance:
Performance of the work will be Offsite. Vendor needs to carry work in their office premises.
