(1) Vendor needs to provide independent facilitation services to individuals that assists participating families and individuals to make informed decisions about what will work best for them, are consistent with their desires and reflect their individual circumstances, independent facilitator isthe person who facilitates the person cantered planning process in collaboration with the person, may offer practical skills t raining to enable families and individuals to remain independent such as providing information on recruiting and hiring personal care workers, managing personal care workers and providing information on effective communication and problem-solving to the government authority located in Flint, MI.
(2) All the questions must be submitted no later than March 26, 2018.

Onshore (USA Only)
[D] Work Performance:
Performance of the work will be Offsite. Vendor needs to carry work in their office location.
