[A] Budget: Looking for Proposals
[B] Scope of Service:
(1) Vendor needs to provide community reintegration health services to the government authority located in Michigan.
- The potential Contractor’s staff must coordinate and/or provide services to paroling and discharging offenders that have been identified during their term of incarceration as having a need that may impede the offender’s successful transition to the community. The Health Services Community Reintegration program (HSCR) is designed to facilitate offenders’ successful transition to the community. Services for the high need offender population may include, but are not limited to, the following services:
1. Housing (Adult Foster Care, Rental Subsidy)
2. Prescriptions
3. Pharmacological Interventions
4. Mental Health Services
5. Medical Services
6. Dental Services
7. Family Interventions
8. Substance Abuse Treatment
9. Employment Services
10. Legal (Guardianship, Advocacy, Fiduciary)
11. Incidental Expenses (Transportation, Toiletries, Clothing, Food Assistance, etc.)
12. Criminogenic needs
13. Crisis response planning
14. Federal and State benefits
(2) All question must be submitted no later than August 23, 2019.

Onshore (USA Only)
[D] Work Performance:
Performance of the work will be Offsite. Vendor needs to carry work in their office location.
