[A] Budget: Looking for Proposals
[B] Scope of Service:
(1) Vendor needs to provide Hosted Health Data Analysis Tools to license/purchase a vendor-hosted, web-based platform to become the hub for our Community Health Assessment and Community Health Improvement Plan, platform must provide the most current population health data on at least 100 indicators from local, state, and national data sources, platform to tell the story of our community’s health, improvement priorities, partnerships, progress and actions, Data visualization tools such as dashboards, maps, trend charts, and disparity graphs must be included, with the option to compare local data to state or nationwide indicators.
- Website Functionality to Community Health Data, Priorities and Action.
(2) A contract period will be one year.
[C] Eligibility:
- Onshore (USA Only);
[D] Work Performance:
Performance of the work will be Offsite. Vendor needs to carry work in their office location.
